By signing below, I agree that Revolution Indoor Cycling, LLC is in no way responsible for the safekeeping of
my personal belongings while I attend class.  I understand that classes at Revolution Indoor Cycling, LLC
may be physically strenuous and I voluntarily participate in them with full knowledge that these activities may
involve potentially dangerous physical activities that may lead to minor injuries or major injuries, up to and
including death. I understand and voluntarily accept full responsibility for the risk of injury or loss arising out
of or related to my participation in Revolution Indoor Cycling LLC classes. I further agree that Revolution
Indoor Cycling, LLC and its agents, members, and representatives will not be liable for any injury including,
without limitation, personal, bodily, or mental injury, disability, death, economic loss or any damage to you,
your spouse or domestic partner, unborn child, heirs, or relatives resulting from the negligent conduct or
omission of any member of Revolution Indoor Cycling LLC or anyone acting on their behalf, whether related
to exercise activities or not.

I agree that neither I, my heirs, assigns or legal representatives will sue or make any other claims of any kind
whatsoever against Revolution Indoor Cycling, LLC or its members for any personal injury, property
damage/loss, or wrongful death, whether caused by negligence or otherwise. To the fullest extent permitted
by law, I hereby forever release, waive and discharge Revolution Indoor Cycling, LLC and its agents,
members, and representatives from any and all claims, demands, injuries, damages, actions or causes of action
related to my participation in Revolution Indoor Cycling LLC classes.

Further, I hereby agree to defend, indemnify, and hold harmless Revolution Indoor Cycling, LLC and its
agents, members, and representatives from and against all liabilities, claims, damages and expenses (including
reasonable attorney’s fees and costs) arising from or related to my participation in classes at Revolution
Indoor Cycling LLC.
